Need assistance with clutch set up....

     I recently trashed a Kennedy stage 2 plate, and I may have over done the adjustment.  When I removed the engine the disc wad just flopping around between the plate and flywheel.  I made sure the clutch adjustment was correct per the Bentley manual.  i'm at a loss as to what could have gone wrong....   Any insight will be appreciated so I don't make the same mistake twice....

Is the main diaphragm spring broken?

  I'm not sure??  I do know that the release cone adaptor is twice as thick and the stage 1 pp that ate a t/o bearing clip.  I'm thinking this is what caused me to over extend the stage 2....
